How to schedule an appointment
Please call and one of our secretaries will book an appointment for you. If you are calling after office hours, please leave a detailed message, including your name and phone number, and someone will return your call on the next business day.
What to bring to your appointment
For all patients:
Please bring your health insurance card, a written list of your prescription medications and any necessary referral information. Additionally, Massachusetts law requires a form of ID for the patient or, if a minor, the guardian will need to have proof of identity.
Copayments
Any applicable copayments and past due balances will be collected at the time of your visit. In addition to cash, checks and debit cards, we accept Master Card, Visa, and Discover. For all returned checks, a $10 fee will be applied in addition to the original amount.
Appointment Cancellation Policy
If your schedule changes and you cannot keep your appointment, please contact us so we may reschedule you, and accommodate those patients who are waiting for an appointment. As a courtesy to our office as well as to those patients who are waiting to schedule with the physician, please give us at least 24 hours’ notice. If you do not cancel or reschedule your appointment with at least 24 hours’ notice, we may assess a $25 “no-show” service charge to your account. This “no-show charge” is not reimbursable by your insurance company. You will be billed directly for it. You can reach the office at (978) 462-7555 option 2 for appointments to reschedule or cancel.
